About the Journal

Clinical Diabetes & Practice Journal is a peer reviewed journal and journal quality is retained by mandatory acceptance of paper by editor and reviewers for publication of an article. The journal publishes most comprehensive, relevant and reliable information based on the current research and development in the field of Diabetes research and clinical practice. The journal ensures barrier-free distribution of its content through online open access.

The journal uses Editorial Manager System for quality in peer review process. Editorial Manager is an online manuscript submission, review and tracking systems. Review processing is performed by the editorial board members of the journal or outside experts; at least two independent reviewers approval followed by editor is required for the acceptance of any citable manuscript. Authors may submit manuscripts and track their progress through the system, hopefully to publication. Reviewers can download manuscripts and submit their opinions to the editor. Editors can manage the whole submission/review/revise/publish process.

Diabetic Peripheral Neuropathy

Diabetic Neuropathy is nerve damage caused by diabetes. When it affects the arms, hands, legs and feet it is known as diabetic peripheral neuropathy. Diabetic peripheral neuropathy is different from peripheral arterial disease (poor circulation), which affects the blood vessels rather than the nerves. The loss of sensation and other problems associated with nerve damage make a patient prone to developing skin ulcers (open sores) that can become infected and may not heal. This serious complication of diabetes can lead to loss of a foot, a leg, or even a life.

Diabetic Amyotrophy

Diabetic Amyotrophy is a disabling illness that is distinct from other forms of diabetic neuropathy. It is characterized by weakness followed by wasting of pelvifemoral muscles, either unilaterally or bilaterally, with associated pain. Sensory impairment is minimal in the cutaneous distribution sharing the same root or peripheral nerve as affected musculature. Electrodiagnostic studies are most often consistent with a neurogenic lesion attributable to a lumbosacral radiculopathy, plexopathy, or proximal crural neuropathy. The natural course of the illness is variable with gradual but often incomplete improvement. The site of the lesion and the pathogenesis of diabetic amyotrophy remain controversial.

Diabetic Nutrition

Diabetic Nutrition refers to the diet that is recommended for people with diabetes mellitus. There is much controversy regarding what that diet should consist of. The diet most often recommended is high in dietary fiber, especially soluble fiber, but low in fat (especially saturated fat) and low in sugar. Recommendations of the fraction of total calories to be obtained from carbohydrate are generally in the range of 40 to 65%, but recommendations can vary as widely as from 16 to 75% . People with diabetes may be encouraged to reduce their intake of carbohydrates that have a high glycemic index (GI), although this is also controversial.

Diabetes Clinical Trials

Clinical trials are an important step in our being able to have new treatments for diabetes and other conditions in order to carefully test a new drug or device in patients before it is approved by the FDA to be used in the general public. The American Diabetes Association is currently a partner providing support for the following clinical studies and initiatives:

• TrialNet Type 1 Diabetes TrialNet is an international network of researchers who are exploring ways to prevent, delay and reverse the progression of type 1 diabetes.

• GRADE GRADE is a comparative effectiveness study looking at what medications work best at lowering blood glucose levels in patients who are newly diagnosed with diabetes.

• RISE The Restoring Insulin Secretion study (RISE) includes 3 studies examining whether aggressive glucose lowering will lead to recovery of pancreas function in those with prediabetes and early type 2 diabetes.

• D2d The goal of the Vitamin D and type 2 diabetes (D2d) study is to determine whether vitamin D supplementation is safe and effective in delaying the onset of type 2 diabetes in people at risk for the disease, and to gain a better understanding of how vitamin D affects glucose metabolism.

Diabetic Neuropathy

Diabetic Neuropathy is a type of nerve damage that can occur if you have diabetes. High blood sugar can injure nerve fibers throughout your body, but diabetic neuropathy most often damages nerves in your legs and feet. Depending on the affected nerves, symptoms of diabetic neuropathy can range from pain and numbness in your extremities to problems with your digestive system, urinary tract, blood vessels and heart. It is a common serious complication of diabetes.

Diabetic Nephropathy

Diabetic Nephropathy is a progressive kidney disease caused by damage to the capillaries in the kidney's glomeruli. It is characterized by nephrotic syndrome and diffuse scarring of the glomeruli. It is due to longstanding diabetes mellitus, and is a prime reason for dialysis in many developed countries. It is classified as a small blood vessel complication of diabetes. The exact reason this occurs in people with diabetes is unknown, but high blood sugar levels and high blood pressure are thought to contribute to diabetic nephropathy.

Diabetic Cardiomyopathy

Diabetic Cardiomyopathy is a disorder of the heart muscle in people with diabetes. It can lead to inability of the heart to circulate blood through the body effectively, a state known as heart failure, with accumulation of fluid in the lungs (pulmonary edema) or legs (peripheral edema). Diabetes mellitus increases the risk  of heart failure independently of underlying coronary artery disease. Several factors may contribute to the development of cardiac dysfunction in the absence of coronary artery disease in diabetes mellitus.

Gastroparesis

Gastroparesis, also called delayed gastric emptying, is a medical condition affecting people with both type 1 and type 2 diabetes in which the stomach takes too long to empty its contents (delayed gastric emptying). The vagus nerve controls the movement of food through the digestive tract. If the vagus nerve is damaged or stops working, the muscles of the stomach and intestines do not work normally, and the movement of food is slowed or stopped. Just as with other types of neuropathy, diabetes can damage the vagus nerve if blood glucose levels remain high over a long period of time. High blood glucose causes chemical changes in nerves and damages the blood vessels that carry oxygen and nutrients to the nerves.

Diabetic Retinopathy

Diabetic Retinopathy, also known as diabetic eye disease, is when damage occurs to the retina due to diabetes. It can eventually lead to blindness. It is an ocular manifestation of diabetes, a systemic disease, which affects up to 80 percent of all patients who have had diabetes for 10 years or more. Diabetic eye disease may include:

• Diabetic retinopathy—damage to the blood vessels in the retina.

• Cataract—clouding of the eye’s lens. Cataracts develop at an earlier age in people with diabetes.

Glaucoma—increase in fluid pressure inside the eye that leads to optic nerve damage and loss of vision. A person with diabetes is nearly twice as likely to get glaucoma as other adults.

Gestational Diabetes

Gestational Diabetes is a condition in which women without previously diagnosed  diabetes exhibit high blood glucose levels during pregnancy (especially during their third trimester). Gestational diabetes is caused when insulin receptors do not function properly. This is likely due to pregnancy-related factors such as the presence of human placental lactogen that interferes with susceptible insulin receptors. This in turn causes inappropriately elevated blood sugar levels.
